Only US made one I know is by Southwest Speed, but reported to be a bit rough looking. I have a lot of Moog parts that are made in Mexico, so my hope is it is of similar quality. Price is right, and it has a warranty.
More research: Global West lists them now ( http://www.globalwest.net/1964-72_A-body_steering_components.htm ) $164.40. Not sure who makes it or where. Hotchkis too, but it looks like you have to buy a rebuild kit to get it ( http://www.hotchkis.net/cart/view_products.php?SubCat=24 )
That was me:bglasses: The Southwest speed one is okay for the $90. Somewhat rough, I spent a few hours smoothing it out with disks to make it look nice. I'd call Ground Up and verify if they do have the RareParts one for $129. Seems like the best deal out there.

As far as I know, RareParts manufactures their own parts. I've also heard they rebuild old units....but that's also expensive. Low-volume or custom work is costly. Especially if it's done in the U.S.
