75 LeSabre Police Car Order Form. Unfortunately its not legible but just want to show a special form from Buick existed.

 

Might want to contact the Sloan Museum and see if they have any wholesale car order form for police cars. There are some police specific trim codes listed on on some order forms:

1962
Trim 484 = Black Vinyl with Bench Front Seat (police option), available on styles 4411 and 4469

1963
Trim 085 = Black Vinyl (police option), available on styles 4019
Trim 484 Black and Silver Vinyl (Police Option), available on styles 4411, and 4496

1964
Trim 092 = Silver Vinyl - Police Option, available on style 4069
Trim 492 = Silver and Black Vinyl - Police Option, available on style 4469
