Looking for a '67 GS 400 order form

I'm looking for a good copy of a '67 GS 400 order form. Anyone have one? I have an electronic copy of one but the resolution is so low that it can't all be read (attached).

You can get a copy of a car order from from the Sloan Longway museum. They have order forms for 1932-1986 for $15 each:
